Under IRC Section 501(r), nonprofit hospitals are legally required to maintain a Financial Assistance Policy and screen patients for eligibility — yet most patients never hear about it before a bill goes to collections. How Charity Care Works

Most hospital Financial Assistance Policies use tiers based on the Federal Poverty Level (FPL): patients under roughly 200% FPL often qualify for 100% forgiveness, with sliding-scale discounts up to 400% FPL or higher depending on the hospital. The exact thresholds are set by each hospital's own policy, not by federal law.
